Office Visits: You will be contacted by our automated patient reminder system to remind you of your appointment. If you are a new patient, please arrive 15 minutes early for paperwork if you have not completed prior to your visit. We ask that you bring all prescription medication with you to your visit.
Hospitalization: When hospitalization is required, a hospitalist will see you at the hospital of your choice.
Referrals: There may be times when the physician must refer you to a specialist. When you are referred to a specialist for consultation and/or testing we will assist you in making those appointments and obtaining pre-authorizations as required by your insurance carrier.
Annual physical/Preventative Health: Our office provides an annual physical exam, annual wellness exams for Medicare patients and most screening test that are covered by your insurance carrier.
Billing: Co-payments are due at the time services are rendered. You will receive a bill from our office after your charges have been processed by your insurance carrier(s). Or if you haven’t heard from your insurance carrier after an extended period of time. It is your responsibility to contact your insurance company if you have not received an “Explanation of Benefits” (or EOB). All non-covered charges, co-insurance and deductibles are due and payable within 30 days of billing. If you do not have insurance and/or special arrangements are necessary for the payment of your bill, please contact our business office to make payment arrangements.
Insurance: Please keep the business office informed of any changes in your insurance status. Although we will assist you in processing your claim, payment of the bill is ultimately your responsibility. We currently participating providers for Aetna, Blue Cross Blue Shield, Bright, Cigna, Cigna Connect, Amerigroup, Community Health Care, Humana, Medicare, Tricare, and United Healthcare. Self Pay as well as many other commercial carriers. Please contact our office for information regarding your health care plan.
